1. 代理记账许可证登记表

2. 机构的协议或者章程。（协议或者章程应载明以下内容：机构名称和地址、业务经营范围，注册资本、法定代表人，内部机构的设置及产生办法，内部机构的职权和议事规则、机构的解散、清算办法以及全体股东签章等事项。）

3. 从业人员身份证明、会计从业资格证书，主管代理记账业务的负责人具备会计师以上专业技术职务资格的证明材料；

4. 主管代理记账业务的负责人，持有会计从业资格证书的专职从业人员在机构专职从业的书面承诺；

5. 办公地址及办公用房产权或者使用权证明；

6. 代理记账业务规范和财务会计管理制度。（代理记账业务规范应包括从业人员职业道德规范、业务操作流程、业务质量控制规范、业务档案管理办法等项制度。）

7. 工商行政管理部门核准机构名称的有关材料，包括企业名称预先核准通知书或企业法人营业执照等
