增值税专用发票丢了怎么办？

第一步

登录国家税务总局福建省电子税务局。需要注意，采用个人实名登录方式。

第二步

进入“我要办税”模块，点击“发票使用”

第三步

点击进入屏幕左侧“丢失增值税专用发票已报税证明”模块

第四步

按丢失的增值税专用发票信息填写表单。

注意，带“*”的项目必须填写，报税日期为该增值税专用发票开票日期。

进入页面右上角“附送资料”按钮上传必报文件，“附送资料”按钮上的红底数字是指“必报”资料数目。

第五步

提交申请后，可回到“我要办税”模块，进入“事项办理”中查询事项进度。

收到申请成功的消息提醒后，即可点击打印《丢失增值税专用发票已报税证明单》
